To download the form to your computer, right-click on "EEE Success Story Template" and select "Save Target As" from the menu. Then navigate to the location on your computer's hard-drive or network where you want to save the file.

To complete the form, use your <Tab> key to move from field to field and then type in your responses. Use your cursor to check the boxes.

Follow the instructions on the form to return it to the Kansas State Department of Education.

 

Success Story Highlight

On Thursday, September 3, 2009 Principal Brenda Anderson of Anderson Elementary School in Tulsa, Oklahoma sent in a success story with more inspiration than imaginable. She wrote, "Quintel Reed is a fourth grader here at Anderson and he is battling brain cancer. He was with us over a year ago when he was diagnosed and took time off for surgeries and chemotherapy. He is still receiving chemotherapy daily. Since his return Mrs. Edwards, a Para, has been taking him to HAT (Healthy Activity Time) every day. I really appreciate Mrs. Edwards for making sure Quintel has the opporutnity to always particpate. However, I thought he wouldn't want to go since he is now wheelchair bound and cannot participate fully. He does enjoy going, and as you will see in the video (see below) he does participate! Quintel's mom happened to be here today for a meeting and when I saw him doing what you will see in the video, I immediately went and got her so she could witness. She came in and said in disbelief that he never moves like that! His mother and his regular education teacher both broke down in tears and had to leave the room, so I took this video on my Blackberry for his mom and wanted to share it with you!

I am very proud of Jasmine and Kenny for incorporating this choreography (to muliplication raps) into their HAT classes. Further, they made it happen in such a way that everyone loved participating - especially Quintel. The students are constantly showing mutual respect and love for Quintel as well, and are always willing to lend a helping hand. Enjoy the video and just remember what an inspiration Quintel is to us all! He truly is an amazing child and we are so proud of him because he will not let his illness stop him! We can all learn from his example!"
